First, as you can see, we have a designated spot for a zip code. This was important because we were only interested in surveying people who lived close to the park, since they are likely to be most impacted by the development. We aimed to make it logical, simple, and one page front and back that would only take 5 to 10 minutes to complete so that participants were not overwhelmed.
